Create a Form Using a Word Processor

You can create a fillable form using a word processor (such as Microsoft Word or OpenOffice.org Writer). You can mail the form by email and the recipient can fill the form save it and email it back to you. You can create surveys, or application forms and any other form.

Steps

OpenOfficeWriter

  1. Open OpenOffice.org Writer
    • In OpenOffice.org Writer click View, Toolbars, Form Control.
  2. Note that this will bring up your forms toolbar. The forms toolbar will have options to add text box, check boxes or radio buttons.
  3. After you create your form make sure you protect your document. Go to tools (and Options then Security in OpenOffice.org) and click on Protect document and choose a password and your document will be protected so that others can't edit your document but they can only fill the text boxes and other fillable options.
  4. Now you can either keep this document on your website or mail it to people to fill it and they can save it and mail it back or print the form.

MS Word 2007

  1. Open Microsoft Word
  2. Click Office button > Word Options.
  3. On Popular tab, enable show developer tab in the ribbon.
  4. Go to developer tab and you can see "Insert Frame" button. You can now insert your frame.
  5. Click Protect Document and select "Restrict Formatting and Editing.
  6. Enter the details of your protection and click the button to implement the protection.
